DOR: Quick Tips and Forms
Referring Participants
We accept all referrals that are in any phase of their UC Berkeley education, and who have graduated no more than a few years ago. We prefer to start working as soon as possible with a participant so that we can connect them with internships and other opportunities along the way.
To Refer
- Send the participant's signed consent forms + IPE to our physical address:

- Upon receipt, we will contact the participant at the address given on the form and schedule an intake appointment.

Our 2-Way Referral Form
We have a referral form |PDF| that we use as a standard way of gathering necessary information about a participant and also for referring a participant to DOR.
It's not necessary for you to send us our referral form if you send us a participant's signed consent form. If, however, you would like to first refer using our referral form and follow up with the signed consent forms + IPE, that's fine. You can have the participant sign our form and fax it to us at (510) 642-6120 (attention: WAIV).
